In recent years, the growth in endovascular procedures has meant that vascular trainees may not gain sufficient experience to be competent in open vascular reconstructions following completion of their training. At the same time many senior surgeons have somewhat limited experience of endovascular techniques. This book will assist junior and senior surgeons and trainees in performing all types of vascular reconstructions. A key feature of the book is its clear illustrations, accompanied by pertinent case examples from vascular registries, so that readers can perform these operations with clear understanding of anatomical exposure and dissection, simplifying the conduct of the procedure.

Reconstructive microvascular surgery is now in its teens. At first many thought this new child was a whim and would fail to thrive. Some were un certain, others with vision either supported or became actively involved in this new area of surgical endeavour. Although initial interest was focused on the replantation of amputated parts, it has been the one stage free trans fer of living tissue to a distant site which has launched microsurgery into the surgical spotlight. From its humble beginnings we have witnessed a revolution in this branch of plastic surgery; many of the long established methods of recon struction have, like barricades, fallen before the advances made in this field. In its infancy there were relatively few procedures available. There was a tendency to make the patient's problem fit the operation, rather than the reverse, and this frequently led to an inferior result. The then known flaps, such as the groin flap and the deltopectoral flap, were employed. Unfortunately they were sites which posed many technical problems; namely those of vascular anomaly, a short pedicle and vessels of small cal ibre. Long operations were the norm, and vascular thrombosis was not un common. Hospital routine often was disrupted and there was a danger that these new techniques would fall into disrepute. Over the last decade this state of affairs has changed dramatically.

Challenging Arterial Reconstructions

Challenging Arterial Reconstructions

Genre: Medical
ISBN: 3030441350

This book presents 100 challenging cases encountered in vascular surgery practice that were selected from the author’s vascular registry of 7,000 vascular reconstructions (endovascular and open). Chapters include difficult surgical problems of patients with aortic, visceral and peripheral arterial aneurysms, with expert management of mycotic, inflammatory and ruptured aneurysms and major complications like aorto-duodenal fistula, graft infection or inferior vena cava injury. These reconstructions are representative of the “real world” experience of vascular surgery in the United States, and the selection of cases were based on either difficulty with case selection, or complex operative or an endovascular technique. The natural history of vascular reconstructions often leads to secondary reconstructions, which are far more complex and therefore are included in significant numbers in this text. Both good and unsatisfactory results have been included and the lessons learned from the poor outcomes are emphasized. Invited commentaries from leading experts in the field have been added to a few chapters offering an alternate viewpoint in the management of patients with complex pathology. Challenging Arterial Reconstructions: 100 Clinical Cases serves as an invaluable resource to vascular surgeons that are learning or regularly performing open surgical or endovascular interventions.

This book offers a comprehensive overview of up-to-date knowledge on vascular access surgery. Written by a senior author with more than 30 years of experience and by expert contributors, it covers both surgical and theoretical aspects. The author shares his expertise in a hands-on approach and presents his views on the difficulties that every clinician may encounter. The various options for vascular access creation are carefully reviewed. Detailed descriptions and numerous accompanying illustrations of AV fistulas, AV prosthetic grafts, and arterio arterial grafts are provided, and a chapter is also devoted to the use of central venous catheters. Potential complications and their management are explained, and advice is offered on how to deal with special patient groups requiring extra attention. Separate chapters on fluid dynamics (drawing on the author’s own research) and vascular pathology cater for the needs of those with a particular interest in the pathophysiological principles.
